Acute Community Care

fluidbagFunded by the Te Whatu Ora, this service is provided by the Acute Community Nursing team (consisting of nurses from Pegasus Health and Nurse Maude). The service is designed to support and assist general practice teams in helping patients who are acutely unwell to remain in their own home for their treatment.  The service is for patients with an acute episode of illness that may otherwise be admitted to hospital. It is for conditions that would usually respond to treatment within 3-5 days, for example cellulitis, urinary retention and respiratory illnesses.

This service allows patients to have the opportunity to recover in their home environment rather than being admitted to hospital, whilst having access to the clinical care and support they need.

You may be referred into this service by your GP, the 24 Hour Surgery doctors or Christchurch Hospital.
